Death records are available for Bath-on-the-Hudson, Greenbush, East Albany and the City of Rensselaer.

The Clerk’s Office only issues Death Certificates for those individuals who have died within our City limits. Certified copies are ONLY issued to the spouse, parent or child of the deceased or to other persons who can provide the following documentation:
-Documented Lawful Right or Claim (An example of a lawful right or claim would be a death record needed by the applicant to claim a benefit. Documentation would consist of an official letter from the agency verifying that to process the claim they require from the applicant a copy of the requested death record.)
-Documented Medical Need
-New York State Court Order

Death records are NOT public records and their confidentiality is protected by New York State Public Health Law. Vital Records are not subject to the provisions of the Freedom of Information Law (F.O.I.L.) and are not subject to public inspection.

The following information is required to obtain a Death Record:
-Name of the Deceased
-Date of Death
-Place of Death
-Relationship of person making request
-Purpose certificate is being requested for

Identification Requirements:
-Driver’s License
-Non-Driver’s I.D.
-Place of Death
-Passport
-Employment I.D.

The fee for each certified copy is $10. If paying by check or money order, please make payable to the Rensselaer City Clerk.
